About The Position

The Discharge Planning Coordinator plays a vital role in ensuring safe, timely, and effective transitions of care for patients. This position supports patients and families by coordinating post-hospital services, addressing social and financial needs, and promoting continuity of care. Requirements

  • High School Diploma or equivalent – required
  • Healthcare, case management, or social services experience preferred
  • Strong communication and interpersonal skills
  • Organizational and time management abilities
  • Critical thinking and problem-solving skills
  • Compassionate, patient-centered approach
  • Ability to manage multiple priorities in a fast-paced environment
  • Compliance with all hospital policies, including vaccination requirements (approved exemptions considered)
  • Successful completion of a criminal background check and pre-employment drug screening (post-offer)

  • Bachelor’s degree in Social Work or related field

Responsibilities

  • Coordinate discharge planning for patients across all age groups
  • Collaborate with physicians, nurses, and interdisciplinary teams to develop appropriate care plans
  • Educate patients and families on post-discharge care, services, and available resources
  • Connect patients with community agencies, home health services, and support programs
  • Assist with financial, social, and placement needs
  • Conduct follow-up to ensure continuity and effectiveness of care plans
  • Support efforts to reduce length of stay and improve patient outcomes
  • Advocate for patients while supporting their emotional and social well-being
