About The Position

Under the supervision of the Manager of Social Services, performs duties as required by government regulation and accreditation standards for discharge planning in collaboration with other disciplines in a hospital setting. Also serves as the alternate Social Service designee, in the absence of the Medical Social Worker, for the Long-Term Care facility.

Requirements

  • High school diploma or GED required.
  • Registered with the Michigan Department of Licensing & Regulation at the minimum level of social work technician or meeting the criteria to become registered within 12 months under supervision.
  • Minimum of two years' experience in providing direct delivery of service to client population.
  • Medical knowledge, or ability to acquire such within one year.
  • Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.
  • Must follow MyMichigan Health and The Michigan Mental Health Code policies and mandatory educational competency requirements.
  • Employees need to demonstrate competency in Microsoft Windows.
  • Ability to handle multiple tasks, get along with others, work independently, maintain regular and predictable attendance, and stay awake.
  • Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.

Responsibilities

  • Coordinates continuum of care arrangements for patients upon referral, working with patients, significant others, physicians, community resources, and other disciplines.
  • Completes a Social Service/Discharge Planning assessment to ascertain patient needs, goals, and resources, including patients/significant others in this process.
  • Reassesses patient needs as appropriate and maintains legible patient records in a timely and confidential manner, ensuring all pertinent forms and related information are organized and accessible.
  • Maintains a daily patient log.
  • Demonstrates awareness of and utilizes available patient resources appropriately, allowing patients freedom of choice where possible.
  • Actively participates in interagency teams and maintains confidentiality.
  • Meets regularly with Social Service organizations for educational, networking, and patient advocacy purposes.
  • Provides essential Social Services to Long Term Care residents in the absence of the Social Worker, including admission protocol, development of discharge plans, participation in care conferences, advocacy, and related documentation.
  • Builds professional relationships with patients, families, and the treatment team to address conflict resolution with positive outcomes.
  • Communicates teamwork, caring, and compassion.
  • Provides excellent customer service in accordance with the mission, vision, and values of the health system.
  • Provides for the age-specific needs of the adult and older adult populations served according to evidence-based standards and organizational policies/procedures.
  • Other duties as assigned.
