About The Position

We are seeking a Discharge Planner, sometimes referred to as a Hospital Discharge Planner, Patient Care Coordinator or a Patient Services Coordinator in our Social Work Services Department. As a Discharge Planner, you will support patients, families, and the healthcare staff. The Discharge Planner work goes beyond clinical care to address the psychosocial, emotional, and practical needs of patients during hospitalization and after discharge.

Requirements

  • Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions, in both managed care and non-managed care environments.
  • Experience in basic to intermediate computer skills/knowledge.
  • High School Diploma required.
  • Behavioral Violence Prevention Training within 3 months of hires and maintain current.

Nice To Haves

  • Current BCLS (AHA) certificate required upon hire and maintain current preferred.
  • Associates degree or LVN license preferred.

Responsibilities

  • Discharge Planning Assistance – interview patients and families, gather necessary information, and help develop safe, patient-focused discharge plans under the guidance of the Case Manager/Social Worker
  • Case Management Support – assist Case Managers/Social Workers by coordinating department activities, handling referrals and authorizations, and managing Medicaid state submission forms for review and filing in accordance with facility procedures
  • Psychosocial Assessments & Emotional Support – evaluate social, emotional, and financial needs while providing crisis intervention, grief counseling, and supportive guidance for patients and families
  • Advocacy & Resource Connection – advocate for patients’ rights and link them with financial aid, housing, transportation, substance abuse programs, and other community resources
  • Interdisciplinary Collaboration & Education – partner with physicians, nurses, and case managers while educating patients and families on treatment plans, insurance, and legal documentation
