Requirements

High School Diploma or GED.
Must possess a combination of behavioral health education and work experience totaling a minimum of four (4) years; or a bachelor’s degree in any field, plus one year of work experience in a behavioral health service delivery; or a bachelor’s degree in a behavioral health field.
Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred.
Valid AZ Driver License.
Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy.
Must have a valid unrestricted Arizona Fingerprint Clearance Card (Level One) or ability to apply for an Arizona Fingerprint Clearance Card within 7 working days of employment with no disqualifying events.
Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
Must pass a typing test with a minimum speed of 25 WPM.
Must pass background check, TB test and other pre-employment screening.

Responsibilities

Serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site.
Work with Level I Inpatient Providers to review presenting clinical information, identify resources for successful discharge to the community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider.
Connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
Work with BHRs while in the hospital, transitioning BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services, reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and TERROS psychiatric provider.
Must be able to work well and tactfully with individuals and groups.
Maintain confidentiality of client records during and after employment at TERROS. Failure to do so may result in dismissal.
