HOPE ~ HEALTH ~ HEALING Seeking a Discharge Planner to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell clinic.

Requirements

  • GED/High school diploma with a minimum of 4 years of behavioral health employment experience, OR An associates degree in the field of behavioral health and 2 years of behavioral health employment experience, OR A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health employment experience, OR A bachelor's degree in the field of behavioral health, OR A master's degree or higher in the field of behavioral health.
  • Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred.
  • Valid AZ Driver License.
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy.
  • Must have a valid unrestricted Arizona Fingerprint Clearance Card (Level One) or ability to apply for an Arizona Fingerprint Clearance Card within 7 working days of employment with no disqualifying events.
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
  • Must pass a typing test with a minimum speed of 25 WPM.
  • Must pass background check, TB test and other pre-employment screening.

Responsibilities

  • Serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site.
  • Work with Level I Inpatient Providers to review presenting clinical information.
  • Review presenting clinical information, identify resources, and help coordinate successful discharge into the community and follow up with the clinical team post discharge.
  • Activities will be performed both in inpatient and outpatient settings (i.e., inpatient visits, coordination meetings, service planning).
  • Connect individuals receiving Level I inpatient services with ongoing Terros Outpatient treatment upon discharge.
  • Work with the clients while in the hospital, hospital unit staff, outpatient team, member family, PCPs, and health plans as clinically indicated to ensure an effective transition.
  • Participation in individualized service planning and execution of written discharge plans.
  • Offer coordination to an array of services that actively address recovery goals.
  • Reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and Terros psychiatric provider.
