About The Position
The Discharge Planner is responsible for performing case management and coordinating the discharge planning process for age-appropriate in-patients with chronic diseases and special health care needs. This includes working within the Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit.
Requirements
- Minimum of 2 years of experience required.
- Valid and unrestricted Registered Nurse license in any U.S. State or Territory
- ACLS
- BLS/BCLS
- PALS
- NRP
- ATT
Responsibilities
- Maintaining a tracking system for referred patients for admission to other healthcare facilities.
- Conducting chart reviews for requested services to facilitate return to facility or discharge home.
