This role provides comprehensive clinical services focused on discharge planning. The Discharge Planner will maintain ongoing communication with the clinical team, regional contacts, families, legal representatives, and referral agencies. The position is responsible for overseeing and participating in difficult patient placements, reviewing discharge plans for rehabilitation patients to ensure PCP appointments are included, and conducting comprehensive evaluations of patient functioning. The role also involves following up with AMA (Against Medical Advice) and no-show patients for aftercare, offering referrals, and documenting patient progress and interventions within regulatory timeframes. Maintaining contact with referents for treatment and discharge planning, providing individual, group, and family treatment, and maintaining linkage agreements with other providers for aftercare services are key responsibilities. The position also participates in quality improvement activities, responds to facility emergencies, and may supervise interns and volunteers. Continuous professional development through continuing education and supervision is expected, along with attendance at daily treatment planning and new patient orientation meetings. The role may involve carrying a caseload of five or more patients depending on facility needs and coordinating presentations from long-term facilities. Specific documentation requirements include D/C plans, PAS 45, and extended day LOCATDRs.
