About The Position

Ridgeview Institute Monroe is seeking a compassionate and skilled Discharge Planner – Social Services to join their team. This full-time role involves performing social services functions to guide patients in accessing treatment skills and achieving optimal emotional health. Responsibilities include offering basic consultation, specialized clinical social work services, crisis intervention, and short-term/long-term family therapy. The Discharge Planner will also provide information and referrals, communicate with referral sources within 24 hours of admission, identify outside professionals working with the patient to facilitate discharge plans, and maintain communication regarding treatment progress and discharge plans. The role also involves interfacing with the Business Development Department, scheduling appointments, communicating with family contacts, assisting with chart audits, and informing patients/families about discharge planning options. The position requires a Bachelor's Degree in Social Work or a related clinical mental health field, with a minimum of one year of related experience, preferably in a hospital setting. CPI training and a competency exam are also required.

Requirements

  • Bachelor's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
  • Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred.
  • CPI training within 30 days of employment and prior to assisting in a restraining procedure.
  • Must be able to pass a competency exam for the area upon hire.
  • May be required to work flexible hours and overtime.

Responsibilities

  • Communicate with referral source within the first 24 hours of admission.
  • Identify all outside professionals currently working with the patient so that appropriate discharge plans can be completed and facilitates appropriate patterns of communication to minimize duplication.
  • Identifies needs of referrals sources and/or professionals during the period of hospitalization.
  • Oversees the tasks of continued communication of treatment progress, discharge plans, and time of discharge to outside interested parties as well as facilitating the transfer of required documents to the outside agents.
  • Interfaces with the Business Development Department to assist with the determination of customer satisfaction and problem identification and solutions.
  • Responsible for discharge plans including scheduling of appointments and communication with parents and significant family contacts.
  • Assists with chart audits and completion of medical records.
  • Inform patients and families on discharge planning options based on diagnoses, prognoses, resources, and preferences related to home care services.
