About The Position

The Discharge Planner assists patients and families with discharge planning by coordinating and implementing post-hospital care. This involves evaluating and assessing patients' needs, financial status/insurance coverage, and available community resources. The Discharge Planner works collaboratively with physicians, Nursing staff, Case Managers/Care Coordinators, patients, and their families/significant others involved in post-hospital care.

Requirements

  • Mature individual, capable of being objective while empathetic and supportive of patients and family's needs.
  • Ability to communicate verbally with all levels of society.
  • Ability to read, write, and speak English fluently.
  • Ability to communicate effectively in Spanish.
  • Ability to work with individuals of all ages, races, and religious backgrounds.
  • Good organizational skills.
  • Ability to remain calm in stressful situations.
  • Basic computer skills.
  • Basic knowledge of community resources.

Nice To Haves

  • Associate of Art or Associate in Science Degree preferred.
  • High school graduate with three to five years' experience in an acute care setting and/or Social Work-related field preferred.
