About The Position
St. Peter's Hospital is searching for a full-time day shift Discharge Planner to join the compassionate Care Coordination team! The Discharge Planner tasks include, but are not limited to the following: Provides discharge planning as a member of an interdisciplinary team. Provides discharge planning, referrals, coordination with community providers and provision of other treatment related duties. Responsible for patient progression of care, interviewing patients for initial discharge plans and coordinating care to the next appropriate site of care.
Requirements
- Bachelor's degree in related field required, or associate degree in related field with QHP required.
- Strong writing and communication skills are required.
Nice To Haves
- Experience in a healthcare/medical setting, preferred.
- Experience working with a diverse population and a strong understanding of multicultural issues is preferred.
