About The Position

We are currently seeking a Full-Time Discharge planner-Case Manager with experience in psychiatric care to join our esteemed team. As a valued member of our organization, you will play a crucial role in enhancing the quality of life for our patients. In this position you would obtain, record, and communicate important clinical information, and provide clinical services within a multidisciplinary team environment. Freedom Behavioral Hospital of Monroe/West Monroe is a 82 bed acute care facility providing specialized care for adults with their mental health and behavioral health needs. The hospital offers a wide range of services, including inpatient and outpatient care, as well as a variety of specialized programs. Requirements

• Experience in psychiatric care
• 1+ years case worker and/or discharge planning activities

Nice To Haves

• Bachelor's Degree - preferably in a Social Services field

Responsibilities

• Assumes responsibility for management of the discharge plan for the patients stay throughout the program.
• Contacts referral sources and family members in order to gather clinical information for the multidisciplinary team and reports to the treatment team.
• Interacts with members of the medical/clinical team to provide a flow of communication and accurately documents in the medical record the findings and data that supports level and intensity of service rendered.
• Functions as a member of the multidisciplinary team and assist in facilitating the treatment team process.
• Works closely with outside agencies to place patients after treatment to manage continuity of care.
• Facilitates communication to outside agencies to ensure that these patient rights, care, and needs are met.
