Discharge Planner/Case Manager - FT | Voyages Conroe
About The Position
The Discharge Planner/Case Manager at Voyages Behavioral Health is responsible for managing the coordination, planning, and placement resolution for all inpatient admissions. This role involves working closely with hospital staff, external agencies, caregivers, and the Department of Family and Protective Services (DFPS) to ensure timely and appropriate discharge plans are developed and implemented. Requirements
- Bachelor’s degree in social work or related field.
- Licensed if applicable in the state in which the hospital resides.
- Current BLS certification required.
- Crisis management certification per hospital policy and standards required.
- Two or more years of related social work or behavioral health experience.
Responsibilities
- Manage the coordination, planning, and placement resolution for all inpatient admissions in the assigned service area.
- Work directly with hospital staff, other agencies, medical consenters, caregivers, and Department of Family and Protective Services (DFPS) to facilitate and coordinate access to behavioral health services and community resources.
- Ensure timely and clinically appropriate discharge plans are developed and implemented.
