About The Position

This role involves providing telephone follow-up for discharged patients. The primary responsibilities include reviewing discharge instructions, confirming necessary appointments, addressing any questions the patient or family may have after leaving the hospital, and referring them to the appropriate person or service if needed. Permission to make these courtesy calls must be obtained before the patient is discharged. The nurse will be accountable for patient satisfaction and service recovery.

Requirements

• Associates in Science required.
• 3 years acute care experience.
• Basic Life Support certification.
• Registered Nurse license.
• Minimum of 3 years of experience required.

Responsibilities

• Provide telephone follow-up for discharged patients.
• Review discharge instructions with patients.
• Confirm required appointments with patients.
• Answer patient/family questions regarding care after hospital discharge.
• Refer patients to the appropriate person/service as needed.
• Obtain permission for courtesy calls prior to patient discharge.
• Demonstrate accountability for patient satisfaction and service recovery.
