About The Position
The Discharge Facilitator facilitates the transition of a patient and family from one environment to another by providing them with a comprehensive continuous plan of care. This includes arranging transportation and preparing information to facilitate the patient's movement to the most appropriate level of care.
Requirements
- BSW, Bachelor's degree, or LPN required
- Screening and assessment skills required
- Expertise in care coordination and/or utilization management in a health care setting required
- Active and current appropriate licensure through the Department of Professional Regulation, State of Florida as indicated
Nice To Haves
- Five or more years of related experience may be substituted for the credentials
