This role serves as a member of an interdisciplinary team, supporting the organization's treatment program and philosophy. Key responsibilities include assisting with discharge planning, coordinating services with other treatment team members, building treatment plans that promote long-term health and relapse prevention, and maintaining effective quality clinical records. The position integrates with all care centers in the hospital, addressing a variety of age-specific concerns, and requires attendance at staff meetings and educational programs. The role also involves participation in Performance Improvement activities and adherence to industry standards and regulatory requirements such as TJC, CMS, HIPAA, and hospital policies. Maintaining a high level of productivity and staying current with clinical trends is essential. The position also includes general office duties.
