About The Position

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. The PACE Program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.

Requirements

  • Bachelor's degree in business administration, healthcare administration, logistics, transportation management, or a related field (or equivalent experience)
  • Several years of transportation, logistics, fleet management, or operations leadership experience
  • Previous supervisory or management experience
  • Strong leadership, organizational, and problem-solving skills
  • Knowledge of transportation regulations, safety standards, and compliance requirements
  • Experience managing budgets and departmental resources
  • Excellent communication and interpersonal skills
  • Proficiency with transportation management and scheduling software

Nice To Haves

  • Experience in a healthcare, senior services, PACE, or human services environment
  • Knowledge of PACE regulations and participant-centered care models
  • Experience managing a fleet of vehicles and transportation staff
  • Lean process improvement or quality improvement experience
  • Familiarity with electronic health records (EHR) and healthcare operations

Responsibilities

  • Provides leadership and oversight for all transportation operations that support participant care.
  • Ensuring participants are transported safely, efficiently, and on time to the PACE center, medical appointments, and other approved services.
  • Develops transportation policies and procedures, manages department budgets, oversees vehicle maintenance and regulatory compliance, and ensures the transportation program meets all PACE and state requirements.
  • Supervises transportation staff, including schedulers, dispatchers, and drivers, providing coaching, performance management, and training.
  • Works closely with the interdisciplinary care team to align transportation services with participant care plans and identify opportunities to improve service delivery.
  • Analyzing transportation metrics, optimizing routes and staffing levels, managing vendor relationships, addressing participant concerns, and implementing strategies to improve efficiency, safety, and customer satisfaction.
