About The Position
Sutton Park Center is seeking a Director of Social Work with subacute and long-term care experience to join its team of professionals. The role is responsible for assessments, care planning, discharge planning, coordination of care and services, and completion and auditing of required clinical documentation. The position is full-time, in person, Monday through Friday on the morning shift, with a salary of $85,000 per year.
Requirements
- Social work license required.
- Strong clinical, communication, and organizational skills.
- Demonstrated judgment regarding safety and confidentiality issues.
- Strong verbal and written communication skills.
- Strong customer service focus.
- Knowledge of community resources.
- Working knowledge of Sigmacare.
- Good working knowledge of long-term care regulations, including documentation requirements.
Nice To Haves
- Bachelor’s or Master of Social Work preferred.
- Prior experience in a skilled nursing facility or long-term care setting preferred.
- Subacute and long-term care experience.
Responsibilities
- Complete MDS assessments.
- Facilitate care plan meetings and discharge planning.
- Implement history and assessment, advance directives, and end-of-life planning.
- Audit psychology paperwork, hospital discharges, room changes, PASRR, and advance directives.
- Complete all required documentation in the medical record (EMR).
- Coordinate care and services, including ongoing monitoring of an appropriate and effective person-centered care plan.
