About The Position

CareOne at Teaneck is seeking a Director of Social Services for their facility. This role involves practicing according to CareOne Social Service Policies and Procedures Manual, State and Federal guidelines, and the National Association of Social Workers & Code of Ethics. The position encompasses supervising social service staff, providing direct services, advocacy, counseling, service coordination, education, policy and program development, and community relations. The Director will also participate in quality improvement initiatives, establish partnerships with the Office of the Ombudsman, and ensure residents and families receive high-quality service in a caring and compassionate atmosphere.

Requirements

  • Master's Degree of Social Work
  • Current and Active LSW (Licensed Social Worker) licensure as required by the State in which you are working.

Responsibilities

  • Supervising social service staff
  • Providing direct services, advocacy, counseling, service coordination, education, in-servicing, policy and program development, and community relations
  • Acting as a resource to other Social Workers
  • Assisting with recruiting, interviewing, hiring, and orienting new personnel
  • Establishing standards of practice and communication channels with other Social Workers
  • Participating in Center-based Quality Improvements
  • Submitting monthly reports to RDO to summarize issues
  • Establishing partnerships with the Office of the Ombudsman
  • Planning and directing a comprehensive social service program
  • Participating in an interdisciplinary team approach
  • Providing therapeutic intervention to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities
  • Maintaining written documentation in the resident's medical record per CareOne policy
  • Performing an initial evaluation/assessment of each resident's psychosocial needs
  • Developing the social service component of the resident's plan of care
  • Participating in and coordinating interdisciplinary care planning conferences
  • Ensuring and documenting that residents/families have input into the care planning process
  • Acting as a liaison between residents, families, outside agencies, and the Center Administrator
  • Conducting family meetings as needed
  • Performing administrative requirements such as completing necessary forms and reports
  • Facilitating discharge planning
  • Ensuring the safety of all Center residents and adherence to legal, safety, health, fire, and sanitation codes
