About The Position

Collaborates with health care team members to identify and provide for each patient/resident’s social, emotional and psychological needs, and the continuing development of the patient/resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge at the Long-Term Care Center/Alzheimer’s Unit, Hospital and Home Health. Assists with emergency situations at the Hospital for family members.

Requirements

  • Bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation, counseling, or psychology
  • One (1) year of supervised social work experience in a health care setting working directly with individuals.
  • Must be able to work efficiently both independently and collaboratively with teams and leadership while demonstrating a high level of integrity, sound judgment, and attention to detail.
  • This role requires professionalism in all interactions with employees, patients, and the public, along with the ability to maintain strict confidentiality and comply with all SECHD, state, and federal policies and regulations.
  • Strong problem-solving, organizational, and time-management skills are essential, as is the ability to manage multiple priorities, adapt to change, and meet deadlines in a fast-paced environment.
  • The candidate must communicate clearly and effectively, both verbally and in writing, and demonstrate strong interpersonal skills with a professional and compassionate demeanor.
  • Computer proficiency and the ability to learn new software systems are required.
  • Dependability, initiative, and the ability to see tasks through to completion are critical, as is a commitment to safety, quality, and working under stressful conditions and/or irregular hours.

Nice To Haves

  • Master’s Degree Social Work
  • Experience working with geriatric population.
  • Knowledge of applicable state and federal regulations is preferred.

Responsibilities

  • Direct and oversee the Social Services Department to ensure quality, resident-centered services that comply with federal, state, and facility regulations.
  • Supervise, mentor, and evaluate department staff; develop and maintain departmental policies, procedures, and quality improvement initiatives.
  • Complete and oversee all required social services documentation, including assessments, MDS, care plans, progress notes, and discharge planning documentation.
  • Assess residents' psychosocial, emotional, behavioral, and family needs and develop individualized interventions as part of the interdisciplinary care planning process.
  • Provide supportive counseling, crisis intervention, advocacy, and education to residents, families, and legal representatives.
  • Coordinate referrals to community resources, mental health providers, support services, and other agencies to meet resident needs.
  • Collaborate with physicians, nursing, rehabilitation, Life Engagement, and other departments to ensure coordinated, comprehensive resident care.
  • Facilitate resident adjustment to the facility, promote resident rights, and advocate for resident dignity, independence, and quality of life.
  • Coordinate discharge planning and transitions of care, including transportation and community service arrangements as appropriate.
  • Maintain ongoing communication with residents, families, legal representatives, and community providers regarding care planning and available resources.
  • Serve as a resource to residents, families, staff, and the community regarding social services, aging, resident rights, and available programs.
  • Investigate incidents, address resident concerns, and participate in performance improvement and risk management activities.
  • Maintain confidentiality of resident and facility information and ensure compliance with HIPAA and all applicable regulations.
  • Protect residents from abuse, neglect, exploitation, and mistreatment, and report concerns in accordance with facility policy and regulatory requirements.
  • Maintain regular attendance, demonstrate professional conduct, and perform other duties as assigned.
