Delivering Solutions that Matter: Provide leadership and oversight for the Social Services department, ensuring residents receive comprehensive psychosocial assessments, care planning, counselling, and ongoing emotional support. Collaborate with interdisciplinary teams, residents, families, and community partners to develop individualized care plans that promote quality of life, dignity, and resident-centered care. Coordinate admissions, discharge planning, crisis intervention, and transitions of care while ensuring compliance with provincial legislation, accreditation standards, and organizational policies. Supervise, mentor, and evaluate social work staff, students, and volunteers, fostering professional development and maintaining high standards of service delivery. Advocate for residents' rights and well-being by facilitating family meetings, addressing concerns, connecting residents with community resources, and supporting programs that enhance social engagement and mental wellness.

Requirements

  • Bachelor or Master of Social Work (BSW or MSW)
  • Current, unencumbered MA Social Worker license
  • Minimum of 1 year prior experience in a Social Worker role, preferably in a healthcare or long-term care, setting.

Nice To Haves

  • Prior supervisory or leadership experience preferred
