Requirements

Bachelor’s Degree in Social Work or related degree in social sciences; licensed where applicable by the state; state requirements may dictate supervision and degree type.
LCSW preferred.
Minimum of one year of social work experience, preferably in a long-term healthcare setting; knowledge of long-term healthcare and public health settings; familiarity with Federal and State regulations and requirements for long-term care.

Responsibilities

Leads and directs Social Service Department.
Manages and directs activities of department in accordance with Volunteers of America National Services’ policies and procedures and guidelines.
Coordinates resident admissions.
Coordinates and leads admission transfer and discharge team.
Completes assessments of residents as necessary for care planning, MDS, RAPS, PPS and other required documents.
Initiate and/or participate in multidisciplinary care conferences held for residents, as appropriate to state and federal guidelines.
Performs other duties or special assignments as directed by the Executive Director.
