About The Position

Westminster Communities of Florida, a faith-based, not-for-profit organization dedicated to serving older adults since 1954, is seeking qualified candidates for the Director of Social Services / Admissions Coordinator position at Westminster Baldwin Park, an Active Living Community in Baldwin Park. This role will support a 20-bed rapid recovery unit. The position is responsible for both Social Services and Admissions for the unit, ensuring residents' emotional and social needs are met and maintained individually, while also marketing the unit to maintain occupancy.

Requirements

  • Bachelor’s Degree in Social Work or Sociology.
  • Three (3) years’ experience in a skilled nursing facility or medically related area is required.

Responsibilities

  • Prepare comprehensive psychosocial assessments and care plans for all residents.
  • During the Admission process, tour the facility and available accommodations with potential residents and family members. Review and obtain signatures for required documentation.
  • Resident and family outreach – advocate for the residents. Main contact for family members.
  • Access community resources and present residents/family members with health care provider availability, including home health care, hospice, and equipment providers.
  • Manage discharge planning; responsible for appropriate discharges to a less restrictive setting (outside health services).
  • Serve as internal contact for resolving resident/family member grievances.
  • As a Grievance Coordinator, maintain a Grievance Log, conduct investigations, and follow up with resolutions. Conduct abuse prevention In-services for staff members.
  • Complete QA and Audits. Monitor psychotropic medications, mood, and behavioral symptoms. Obtain consent from residents or HC Surrogate for psych med use and maintain in the psychoactive medications binder.
  • Coordinate all admissions with transfers from hospital or home to facility.
  • Tour the facility with prospective residents/family members. Assist with referrals as needed.
  • Assist in the evaluation of all potential residents to ensure appropriate placement.
  • Ensure proper federal, state, and facility-mandated documentation is completed. Ensure all necessary Medicare and/or insurance information is available and confirmed prior to admission.
  • Review admission packets for accuracy and approval. Enter new resident information into the Electronic Charting System.
  • Educate residents and families about the extent of Medicare and Medicaid coverage, including length of stay, amount of coverage, and co-insurance days.
  • Develop and maintain relations with physicians and hospitals on an ongoing basis, including providing them with Medicare updates and changes in facility procedures.
