About The Position

This position requires good interpersonal skills, and the ability to communicate clearly with others. Respond to the emotional needs of residents through regular visits, and social group work. Provide support to resident’s families.

Requirements

  • Licensed Social Worker in the State of Alabama
  • Knowledge of state/federal regulations for Social Service in a long term care facility
  • Continued education seminar participation in related areas

Responsibilities

  • Respond to the emotional needs of residents through regular visits, and social group work.
  • Provide support to resident’s families.
  • Receive admission material, and gather appropriate medical and social information.
  • Participate in In-Home or In-Hospital pre-admission assessments when needed.
  • Obtain, and document complete and relevant social histories on all residents.
  • Document Social Service contacts, and intervention in resident chart.
  • Develop Social Service Care Plans and goals for each resident.
  • Update progress notes as mandated by state/federal requirements.
  • Provide consultation, support, and on-going contact with residents, families, and staff.
  • Provide individual support services to residents on a regular basis.
  • Be available for, and become involved in crisis intervention when needed.
  • Insure that communication is maintained between hospital and facility staff during resident hospitalization pertaining to resident status and readmission.
  • Contact families regarding resident problems, resident personal needs, transfers, and change of level assessment.
  • Develop a working team approach with all levels of personnel, maintaining professionalism at a management level.
  • Participate in multi-disciplinary patient care conference to help develop a team approach in meeting the residents’ needs.
  • Serve on committees, and attend meetings, i.e., department head meetings, etc.
  • Participate in the Family Council Meetings held quarterly.
  • Serve as discharge coordinator, and assist residents, and families to find community support services, or alternative living arrangements if needed.
  • Provide In-Service training to all relevant personnel regarding resident’s rights, to help ensure residents are treated with respect, dignity, death/dying advance directives, and other pertinent education to enhance the staff’s knowledge and ability in meeting resident’s psychosocial needs.
  • Keep lines of communication current, and flowing at all times, to all facility personnel, Residents, families, and community.
  • Performs other duties as assigned.
