Relient Health is currently seeking an experienced Occupational Therapist Director of Rehabilitation (DOR) to lead a respected Skilled Nursing Facility (SNF) in Concord, CA. This opening is for a full-time, direct-hire leadership opportunity for a seasoned therapy professional with proven experience in managing rehabilitation operations in a Skilled Nursing Facility (SNF) setting. The facility is a stable, mission-driven Skilled Nursing Facility (SNF) known for quality therapy outcomes and a strong interdisciplinary team. The DOR will lead PT, OT, and SLP services, ensuring clinical excellence, efficient operations, and a supportive environment for both patients and staff.

Requirements

Active Occupational Therapist (OT) license in CA
Minimum 1 year of recent experience as a DOR in a Skilled Nursing Facility (SNF) setting or equivalent (REQUIRED)
Strong communication and leadership skills
Knowledge of Medicare documentation, compliance, and billing standards

This position is open only to licensed therapy professionals who already have experience as a Director of Rehab in a Skilled Nursing Facility (SNF) setting.
We are not seeking clinicians (PT, PTA, OT, COTA, or SLP) who are looking to transition into their first management role.
