About The Position

The Director of Five-Star & Quality Performance is responsible for leading organization-wide initiatives that drive Five-Star ratings, Quality Measures performance, MDS accuracy, and quality incentive outcomes across multiple skilled nursing facilities. This role serves as the subject matter expert for Five-Star methodology, MDS processes, Quality Measures, and the Ohio Quality Incentive Program (QIP), partnering closely with facility leadership teams to improve clinical outcomes, regulatory performance, reimbursement accuracy, and overall quality ratings. This is a primarily remote position with regular travel to assigned facilities and corporate offices. Candidates should be located in the Columbus or Cincinnati market. Candidates based in Cincinnati must be willing to travel overnight as needed to facilities in Northern Ohio, including the Cleveland and Toledo areas.

Requirements

  • Current Registered Nurse (RN) license required; exceptional Licensed Practical Nurses (LPNs) with significant MDS and Quality Measure expertise will be considered.
  • Minimum of 5 years of multi-site skilled nursing or long-term care experience.
  • Minimum of 5 years of MDS experience.
  • Demonstrated expertise in CMS Five-Star methodologies and Quality Measures.
  • Experience with the Ohio Quality Incentive Program (QIP).
  • Strong analytical skills with the ability to interpret clinical, quality, and reimbursement data.
  • Proven ability to influence and lead change across multiple facilities.
  • Excellent communication, presentation, and coaching skills.

Nice To Haves

  • RAC-CT certification or similar reimbursement certification.
  • Experience supporting multiple skilled nursing facilities in a regional or corporate role.
  • Experience leading quality improvement initiatives resulting in measurable Five-Star or Quality Measure improvements.

Responsibilities

  • Lead organization-wide strategy to improve and sustain CMS Five-Star ratings across multiple facilities.
  • Monitor and analyze Five-Star performance metrics, identifying trends, risks, and opportunities for improvement.
  • Develop and implement action plans to improve Health Inspection, Quality Measure, and Staffing domains.
  • Provide ongoing education and coaching to facility leadership teams regarding Five-Star methodology and performance.
  • Serve as the organization's expert on Quality Measures and quality reporting programs.
  • Analyze facility-level Quality Measure performance and develop improvement strategies.
  • Conduct routine audits to validate data integrity and identify opportunities for enhanced outcomes.
  • Partner with clinical and operational leaders to improve resident outcomes and overall quality performance.
  • Conduct monthly calls with the centers to review current metrics and project metrics for end of quarter/ MDS.
  • Collaborate with facility MDS teams to improve coding accuracy and Quality Measure outcomes.
  • Support education and competency development of MDS coordinators and interdisciplinary teams.
  • Lead organizational strategies related to Ohio's Quality Incentive Program.
  • Monitor facility performance against QIP benchmarks and performance measures.
  • Identify opportunities to maximize incentive payments through improved quality outcomes and compliance.
  • Educate facility leaders on QIP requirements and financial impact.
  • Provide consultation, coaching, and support to administrators, DONs, MDS coordinators, and regional leadership as related to the company’s quality initiatives.
  • Input center’s percentages into our projection applications and reports to track quality initiatives across facilities.
  • Participate in facility performance reviews and strategic planning meetings.
  • Assist with survey preparedness and post-survey quality improvement efforts.
