About The Position

A growing home care agency is seeking a strong, hands-on Director of Patient Services (DPS) to oversee clinical operations, compliance, nursing oversight, and patient care quality across the organization. This is a high-impact role for someone who takes ownership, operates proactively, and can confidently manage the clinical department independently. The ideal candidate is organized, accountable, solutions-oriented, and experienced in ensuring visits, documentation, audits, and patient care are handled accurately, efficiently, and on time.

Requirements

  • Home care experience required
  • Experience with audits, compliance, DOH surveys, and regulatory standards
  • Experience supervising nurses or clinical teams
  • Strong knowledge of Medicaid, Medicare, and NY home care regulations
  • Strong leadership, communication, organizational, and problem-solving skills

Nice To Haves

  • Active RN license preferred
  • Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred
  • Experience with HHAeXchange, Sandata, RN Pad, or similar systems preferred
  • Bilingual Spanish preferred, but not required
  • Takes ownership and leads independently
  • Comfortable making decisions and solving problems proactively
  • Strong accountability and follow-through
  • Team-oriented with a positive attitude and professionalism
  • Flexible to occasionally visit patients in the field as needed

Responsibilities

  • Oversee day-to-day clinical operations, patient care plans, assessments, and service delivery
  • Review and approve assessments, Plans of Care, MD Orders, and clinical documentation
  • Conduct home visits, reassessments, UAS assessments, and event-based visits as needed
  • Ensure compliance with NYSDOH, Medicaid, Medicare, and home care regulations
  • Prepare for DOH audits, managed care audits, inspections, and surveys
  • Monitor patient outcomes, incidents, complaints, caregiver performance, and service delays
  • Lead Quality Improvement initiatives, meetings, and corrective action plans
  • Maintain audit-ready compliance binders, logs, and documentation
  • Supervise, train, coach, and evaluate nurses and outsourced clinical vendors
  • Ensure outsourced nursing visits and communication are handled properly and on time
  • Work closely with HR, Intake, Coordination, physicians, and leadership teams
  • Hire, train, write up, and manage staff within the department
  • Track KPIs and implement operational and clinical improvements
  • Assist with staffing, budgeting, and strategic growth initiatives
