About The Position

Join our team as Director of Patient Care Services and lead clinical excellence across our home health services. We are seeking an experienced and compassionate Registered Nurse leader to serve as our Director of Patient Care Services. In this key leadership role, you will provide clinical direction and operational oversight while ensuring patients receive safe, high-quality, compassionate, and compliant care. You will work closely with clinical, operations, quality, and leadership teams to drive strong patient outcomes, support and develop clinicians, maintain regulatory readiness, and create a positive experience for both patients and employees.

Requirements

  • Current Registered Nurse (RN) license in the state of practice.
  • Current CPR certification.
  • Minimum of 2 years of home health experience.
  • Clinical leadership, management, or supervisory experience preferred.
  • Strong knowledge of home health clinical practices, regulatory requirements, and quality standards.
  • Strong communication, organizational, leadership, and problem-solving skills.
  • Ability to effectively coach and support clinical teams while maintaining accountability for performance and outcomes.

Responsibilities

  • Provide clinical leadership and oversight of daily home health patient care operations.
  • Ensure care is delivered in accordance with professional standards, agency policies, and applicable state and federal regulations.
  • Oversee the coordination and management of patient care from referral and admission through ongoing assessment, care planning, transfers, and discharge.
  • Ensure individualized plans of care are developed, implemented, evaluated, and appropriately documented.
  • Review and approve OASIS documentation as required.
  • Supervise, coach, mentor, and develop clinical leaders and field staff to promote competency, accountability, and professional growth.
  • Ensure clinicians maintain required licensure, training, competencies, and compliance standards.
  • Lead quality and compliance initiatives, including QAPI, incident investigations, corrective action plans, accreditation, survey readiness, and audit response.
  • Monitor clinical performance, patient outcomes, patient satisfaction, and workflow metrics and develop improvement strategies when needed.
  • Partner with operations and leadership on staffing, productivity, resource allocation, and on-call coverage.
  • Provide clinical leadership during emergency preparedness planning, response, and post-event review.
  • Serve as a clinical resource and escalation point for complex patient, caregiver, and staff concerns.
