About The Position

The Director of Palliative Care Services provides oversight of the PHC Enhanced Palliative Care Program, its clinical and administrative operations, and all associated staff. This includes palliative care activity in IPU and ambulatory care center(s) as well as home and community based settings of care. The Director ensures the clinical and operational alignment of various affiliate and associate palliative care programs within the network, including seamless integration with telehealth/telemonitoring/RPM technologies and transitional supports/care management. The Director of Palliative Care Services works with leadership across the enterprise to drive quality and outcomes, standardization, and develop strategies to increase the capacity for at-risk populations served in order to meet the strategic goals of Pure Healthcare.

Requirements

  • Minimum of five years clinical leadership experience.
  • Bachelor's Degree in Nursing, Master's preferred
  • Proficiency in Quality Improvement initiative and project management
  • Computer skills sufficient to properly document services and analyze data including Microsoft Office 365, Teams, Power Bl
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage

Responsibilities

  • Creates, standardizes, and maintains a high quality, cost-effective system to deliver Enhanced Palliative Care in all settings.
  • Coordinates, plans, implements, and evaluates evidence-based care in accordance with the organizations mission, vision and strategic objectives.
  • Develops a standardized model of palliative care and implements protocols, systems, and processes that support operations while ensuring quality care.
  • Serves as a liaison to the community in marketing palliative care services and participates in organizational marketing and community activities to promote organizational growth.
  • Works directly with key stakeholders (i.e., payers, partners, governments, and health systems) to implement the Pure Healthcare model of care
  • Expands palliative services to support at-risk populations and alternative payer arrangements and assures the achievement of target clinical outcomes.
  • Utilizes clinical data, operational data, patient satisfaction data and other forms of information to improve systems of care.
  • Supervises clinical practice of staff through rounds, in-person and virtual communication to ensure quality of patient care, safety, assessment of documentation and execution of responsibilities
  • Participates with leadership in continuous quality improvement, development of quality plans and reporting system specific to populations served.
  • Collaborates with leadership to create, implement, and evaluate written policies and procedures.
  • Evaluates product service line performance in financial, productivity, marketing and quality data and provides feedback for change to maintain organizational growth.
  • Evaluates customer satisfaction of product service line and contributes to the resolution of action plans for customer service concerns.
  • Assist in other activities and departments when requested to ensure model alignment.
  • Participate in Continuous Quality Improvement activities to support the quality of services
