About The Position

Assisting Hands Home Care is seeking a compassionate, organized, and experienced Registered Nurse (RN) to join their leadership team as the Director of Nursing in Bergen County, NJ. Requirements

Active New Jersey RN License
Strong communication and interpersonal skills
Compassionate and professional bedside manner
Organized with strong follow-through
Comfortable working independently and making decisions
Reliable transportation required

Nice To Haves

Home care experience preferred
Management or supervisory experience preferred
Experience with electronic documentation systems preferred

Responsibilities

Conduct client assessments and reassessments in the home
Develop and update individualized care plans
Perform supervisory visits for caregivers and clients
Support new client start of care processes
Assist with caregiver training and clinical guidance
Communicate with families, hospitals, hospice teams, rehab facilities, and referral sources
Help resolve client concerns and service issues
Participate in care coordination and case management
Monitor client condition changes and document appropriately
Assist with incident follow-up and quality assurance
Work closely with office staff, schedulers, and leadership team to maintain excellent client care
