About The Position

Luxe Hospice and Palliative Care is dedicated to providing compassionate, high-quality care to patients and their families during their most critical moments. We are currently seeking a skilled and compassionate Hospice Director of Nursing to join our team. As a Hospice Director of Nursing, you will be responsible for coordinating the care of hospice patients and ensuring they receive the highest level of care throughout their journey. The ideal candidate will have hospice experience and the ability to develop and implement individualized care plans in collaboration with the interdisciplinary team. This role requires a compassionate approach to patient care, attention to detail, and strong communication skills.

Requirements

Previous experience in hospice care is required.
Registered Nurse (RN) with current licensure is required.
Strong organizational and time management skills.
Compassionate, patient-centered approach to care.
Excellent communication and interpersonal skills.
Hospice: 3 years (Required)

Responsibilities

Assessing patients’ needs, developing care plans, and coordinating care to ensure that patients and their families receive the necessary support.
Collaborating with physicians, clinical staff, and other members of the interdisciplinary team to provide holistic care that addresses the physical, emotional, and spiritual needs of patients.
Regularly evaluating the patient’s condition, updating care plans, and ensuring compliance with hospice regulations and company policies.
Maintaining patient records and documentation in compliance with company and regulatory standards.
