About The Position

The San Francisco Campus for Jewish Living (SFCJL), a mission-driven nonprofit organization founded in 1871 and rooted in Jewish values, is seeking a Director of Memory Care. Located in San Francisco's Excelsior neighborhood, SFCJL provides a continuum of care for older adults, including luxurious assisted living and memory care services, long-term skilled nursing, rehabilitation, and specialized mental healthcare. Requirements

  • Active California Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license required; BSN preferred.
  • Minimum of five (5) to seven (7) years of clinical experience, with a strong preference for candidates who have worked within an assisted living or memory care setting.
  • Strong knowledge of state, federal, and DSS regulations.
  • Demonstrated leadership and staff development experience.
  • Ability to manage and coordinate a multi-faceted clinical program, including problem resolution.
  • Strong analytical and decision-making skills with the ability to respond quickly in clinical situations.
  • Excellent organizational, interpersonal, and communication skills.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.
  • Strong attention to detail and accuracy.
  • Proficiency in computer systems, including documentation platforms and Microsoft Office.
  • Knowledge of quality improvement, infection control, risk management, and survey processes preferred.
  • Ability to maintain confidentiality and communicate effectively across all levels of the organization.
  • Frequent standing, stooping, bending, stretching, and squatting.
  • Ability to stand and walk for up to 70% of the workday.
  • Ability to work on a computer for extended periods.
  • Ability to lift, transfer, turn, and position residents weighing up to 125 pounds unassisted, or with assistance for residents over 125 pounds.
  • Ability to lift and carry up to 25 pounds frequently and 50 pounds occasionally.
  • Ability to work in environments with potential exposure to communicable diseases, odors, chemicals, and varying temperatures.
  • Ability to maintain focus and attention to detail in a fast-paced environment with frequent interruptions.
  • Fine motor skills with good visual acuity.

Responsibilities

  • Provide leadership, oversight, and coordination of all memory care services to ensure a safe, engaging, and high-quality environment for residents living with cognitive impairment.
  • Promote person-centered care, ensure regulatory compliance, supervise staff, and partner with families and interdisciplinary teams to support residents’ physical, emotional, and psychosocial well-being.
  • In the absence of the Director of Health and Wellness, assume full operational oversight of community health services and wellness programming to ensure continuity of care and compliance.
  • Serve as the clinical leader, supporting the care team and overall healthcare operations of the community.
  • Collaborate with clinical and non-clinical staff to meet the healthcare needs of residents.
  • Hire, train, supervise, and manage caregiving staff, including caregivers and medication technicians.
  • Ensure all healthcare operations comply with DSS regulations, including incident reporting and required in-service training programs.
  • Conduct pre-admission assessments, appraisals, and re-appraisals in accordance with regulatory requirements.
  • Develop, implement, and maintain individualized care and service plans, including updates based on changes in resident condition.
  • Monitor and anticipate changes in resident status and ensure appropriate interventions.
  • Lead clinical quality and process improvement initiatives through tracking, trending, and analysis of internal data.
  • Oversee contracted pharmacy operations, ensuring compliance with medication management regulations, including storage, administration, and documentation.
  • Coordinate with external healthcare providers, including hospitals, physicians, home health agencies, hospice providers, and specialty clinics.
  • Represent the community as a healthcare professional in external partnerships and professional interactions.
  • Coordinate with internal transportation services to support resident medical appointments.
  • Respond to emergencies in accordance with established emergency management procedures.
  • Participate in overall community operations and events as needed.
  • Serve as a liaison to residents’ families, addressing concerns, requests, and care needs.
  • Collaborate with caregiving staff to develop and manage staffing schedules.
  • Provide direct clinical and caregiving support to residents when needed.
  • Perform other duties as assigned.
