About The Position

Serenity Hospice is seeking an experienced and compassionate Director of the Encore Program to lead the growth of our specialized memory care and dementia services across our Pennsylvania locations. This is a unique opportunity to take your dementia expertise beyond traditional bedside care. You will help shape a program that supports patients living with Alzheimer’s disease and dementia through comfort-focused hospice care, meaningful engagement, behavioral understanding, and compassionate family support. Requirements

Certified Dementia Practitioner (CDP) certification or equivalent dementia-focused training
3–5+ years of experience working with advanced dementia or memory care populations
Experience with education, training, or program development
Strong leadership, communication, and presentation skills
A compassionate and patient-centered approach
A passion for educating others and improving dementia care

Responsibilities

Develop and standardize dementia-focused care practices
Educate interdisciplinary hospice teams
Provide guidance and education to families
Help integrate meaningful sensory and non-pharmacological interventions into patient care
Collaborate with Clinical Directors and Patient Relations Managers to introduce the Encore Program to memory care communities, assisted living facilities, skilled nursing facilities, and other healthcare partners throughout Pennsylvania
