In this role, you’ll lead and support quality improvement initiatives that enhance resident outcomes, regulatory compliance, and clinical excellence. Requirements

  • Associate's Degree in Nursing
  • Current New York State Registered Nurse (RN) License required
  • Strong knowledge of CMS regulations, NYSDOH requirements, QAPI principles, and the survey process
  • Experience with clinical auditing, data analysis, and quality improvement initiatives
  • Excellent communication, organizational, leadership, and problem-solving skills
  • Ability to analyze data, identify trends, and implement performance improvement strategies
  • Strong attention to detail and commitment to regulatory compliance and resident-centered care
  • Ability to collaborate effectively with interdisciplinary teams and facility leadership

Nice To Haves

  • Bachelor's Degree in Nursing preferred
  • Long-term care or skilled nursing experience preferred

Responsibilities

  • Lead and monitor Quality Assurance and Performance Improvement (QAPI) initiatives throughout the facility
  • Conduct clinical audits to ensure compliance with CMS, NYSDOH, federal regulations, and facility policies
  • Track, trend, and analyze quality indicators including falls, infections, rehospitalizations, pressure injuries, and other clinical outcomes
  • Collaborate with nursing leadership and interdisciplinary teams to develop and implement corrective action plans
  • Identify opportunities for process improvement and support implementation of best practices
  • Assist with survey readiness activities and support regulatory inspections and audits
  • Educate and mentor staff on quality measures, documentation standards, regulatory requirements, and clinical best practices
  • Participate in root cause analyses and performance improvement projects
  • Monitor quality metrics and provide recommendations to improve resident outcomes and operational performance
  • Maintain accurate records, reports, and quality improvement documentation
  • Perform additional duties as assigned
