About The Position

We are seeking a strong, hands-on Director of Care Coordination to oversee the coordination department within a growing home care agency. This role is responsible for managing coordinators, ensuring patients are properly staffed and serviced, driving accountability across the team, and keeping operations running smoothly. We are looking for someone proactive, organized, bilingual in English/Spanish, and capable of taking full ownership of the department.

Requirements

  • Bilingual English/Spanish required
  • Minimum 3–5 years of home care experience, preferably within a NYS LHCSA
  • At least 2 years of supervisory or management experience
  • Strong knowledge of NYSDOH regulations and LHCSA operations
  • Strong leadership, management, and decision-making skills
  • Excellent organizational and analytical abilities
  • Proficiency in EVV systems and home care software (HHAeXchange preferred)
  • Strong communication and interpersonal skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Ability to work independently and take ownership

Responsibilities

  • Oversee daily coordination operations and Care Coordinators
  • Ensure continuity of care and proper staffing coverage
  • Monitor KPIs, EVV compliance, utilization, attendance, and service quality
  • Identify operational issues and proactively solve problems
  • Coordinate with Intake, HR, Payroll, Billing, Recruitment, and DPS
  • Hire, train, coach, and manage coordination staff
  • Hold team members accountable for performance and follow-through
  • Improve workflows, communication, and operational efficiency
  • Step in operationally when needed to support the team
  • Maintain compliance with NYSDOH and LHCSA regulations
