The Director of Clinical Services (DCS) is responsible for the overall direction of home health and hospice clinical services. The DCS establishes implements and evaluates goals and objectives for home health and hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. Signature Healthcare at Home provides professional, home-based medical care for patients needing rehabilitation and expert care following surgery, a hospital stay or other illness. Requirements

  • Overall direction of home health and hospice clinical services.
  • Establishment, implementation, and evaluation of goals and objectives for home health and hospice services.
  • Ensuring compliance with state and federal home health and hospice laws and regulations.
  • Coordination and oversight of all direct and indirect patient services.
  • Development, implementation, and updates of individualized plans of care.
  • Guidance and counseling to coordinators and Clinical Supervisors.
  • Assistance to Clinical Supervisors in managing clinical teams and planning.
  • Support in assessment, planning, implementation, and evaluation of patient and family/caregiver care.
  • Assistance to the Executive Director/Administrator in budget preparation and administration.
  • Interpretation of operational indicators to detect census changes and impact on staffing, revenues, or expenses.
  • Evaluation of performance of Clinical Supervisors and all clinical personnel.
  • Assistance to Clinical Supervisors in developing skills for evaluating clinician performance.
  • Hiring, evaluation, and termination of organization personnel.
  • Conducting clinician performance evaluations.
  • Oversight of patient clinical records maintenance.
  • Assurance of proper maintenance of clinical records in compliance with laws.
  • Maintenance of adequate inventory of supplies and equipment.
  • Development, implementation, and evaluation of orientation programs for new personnel.
  • Planning and implementation of in-service and continuing education programs.
  • Assistance with evaluation of organization performance.
  • Assurance of quality and safe delivery of home health and hospice services.
  • Assistance in development of organization goals.
  • Development, recommendation, and administration of Organization policies and procedures.
  • Assurance of compliance with licensure and certification laws for personnel.
  • Staying informed about changes in nursing and home health and hospice care.
  • Promotion of home health and hospice referrals.
  • Acting Executive Director/Administrator in the absence of the Executive Director/Administrator.

Responsibilities

  • Ensures Agency compliance with all applicable state and federal home health and hospice laws and regulations.
  • Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel. Ensures a Clinical Manager/Supervisor is available during all operating hours.
  • Responsible for assuring the development, implementation, and updates of the individualized plan of care, which would entail communication with all physicians involved in the plan of care and integration of orders, including medication orders, from all physicians involved in the plan of care.
  • Provides guidance and counseling to coordinators and Clinical Supervisor to assist them in continually improving all aspects of home health care services, provided through organization personnel.
  • Assists Clinical Supervisors in managing clinical teams and planning.
  • Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
  • Assists the Executive Director/Administrator in the preparation and administration of the organization's budget.
  • Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses.
  • Evaluates performance of Clinical Supervisors and all clinical personnel.
  • Assists Clinical Supervisors to develop skills and techniques in evaluating the performance of clinicians.
  • Hires, evaluates, and terminates organization personnel.
  • Conducts clinician performance evaluations annually, or more frequently if indicated.
  • Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.
  • Develops implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, either directly or by delegating to another staff member.
  • Plans and implements in-service and continuing education programs to meet education and training needs of organization personnel.
  • Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews.
  • Assures for the quality and safe delivery of home health and hospice services provided through the Organization.
  • Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures.
  • Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance.
  • Stays informed about changes in the field of nursing and home health and hospice care; shares information with appropriate organization personnel.
  • Promotes home health and hospice referrals in the health care community.
  • In the absence of the Executive Director/Administrator, the Director of Clinical Services will become the acting Executive Director/Administrator and will be vested with authority to act in behalf of the Executive Director/Administrator.
  • Other duties as assigned by the Executive Director/Administrator.
