About The Position

As a Director of Clinical Management at CenterWell Home Health, reporting to the Branch Director, you will direct clinical services provided by the branch. Ensure adherence to federal and state accreditation and company standards for the provision of reimbursable patient services, and compliant clinical records substantiating those services. As a Registered Nurse Clinical Manager, you will: Acts as alternate administrator when BD is not available and shares responsibility with the BD for seeing that regulations and standards are met. Participates in developing objectives and standards for the HHA which ensure safe and therapeutically effective services to patients and families. Consults with BD to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of clinical personnel. Participates in the recruitment, interviewing, selection and orientation of staff members, evaluates their performance relative to job goals/requirements, coaches them and ensures adherence to internal policies. Investigates and reports any problems relating to patient care or conditions which might harm the patient and/or employee well being. Monitors supplies/equipment for appropriate use and ordering and takes steps to minimize misuse. Ensures compliance with infection control standards, including the tracking and trending of employee/patient infections and manages bio-hazardous waste program. Ensures that patients’ plans of care are developed, implemented and evaluated. Conducts monthly staff meetings and/or in services in collaboration with BD. Oversees completion of clinical record reviews and PAE’s timely. Completes clinical portions of QAPI in collaboration with BD. Ensures adherence to PI plans. Oversees scheduling and completion of annual supervisory visits for all field staff. Ensures service quality and participates in care coordination to ensure proper communication between caregivers, patients, referral sources and payors. Discuss operational issues, updates staff on new/changed regulations and reviews records/documentation to ensure regulatory and in-house compliance. Available to share admin on call rotation as needed; able to perform nursing functions in times of emergent staff shortage. Participates in special projects and performs other duties as required.

Requirements

  • Current state license as a Registered Nurse.
  • Minimum of 5 years of clinical experience in nursing including acute/home care experience in the past 2 years and 2 years in a supervisory/administrative role.
  • Expert knowledge of home health care supervision, home health agency regulations and CMS services required.
  • Excellent organizational, problem solving, decision making and communication skills are required.
  • Proof of current CPR.
  • Valid driver's license, auto insurance and reliable transportation.
  • Will be required to be screened for TB as part of Humana's Tuberculosis (TB) screening program.
  • Must have a valid state driver's license and maintain personal vehicle liability insurance with minimum coverage of $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage, or whichever is higher.
