About The Position

We are seeking a seasoned Director of Case Management to provide strategic and operational leadership for a large, complex acute care hospital in Las Vegas, Nevada. This executive role oversees hospital-wide case management, care coordination, social work, and related services, ensuring efficient patient throughput, regulatory compliance, and high-quality discharge planning. This position requires a confident, data-driven leader with strong interdisciplinary influence, the ability to lead large teams, and a proven record of driving performance improvement in acute care environments.

Requirements

  • Bachelor’s degree.
  • One of the following active licenses: Registered Nurse (RN), or Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW).
  • Minimum of 3 years of acute care hospital case management experience.
  • Minimum of 2 years of case management leadership experience.
  • Demonstrated ability to analyze data, develop action plans, and execute operational improvements.

Nice To Haves

  • Master’s degree in Nursing, Health Administration, or Business Administration.
  • Experience leading large, multidisciplinary teams in complex acute care settings.
  • Strong background in utilization management, patient flow optimization, and physician collaboration.

Responsibilities

  • Direct and oversee all aspects of hospital case management, care coordination, discharge planning, and social work services.
  • Lead departmental strategy, staffing models, skill mix, onboarding, and competency management to meet quality, financial, and operational goals.
  • Ensure timely, compliant, and high-quality care coordination and discharge planning processes across all patient populations.
  • Monitor department performance through data analysis, productivity metrics, and outcome measures; implement sustainable performance improvement initiatives.
  • Allocate resources effectively to meet patient flow, length-of-stay, and utilization management objectives.
  • Maintain compliance with federal, state, and regulatory standards related to case management practice.
  • Collaborate with executive leadership, medical staff, nursing leadership, finance, legal, ethics, and post-acute partners.
  • Build and maintain strong relationships with physicians and community-based post-acute providers.
  • Lead, mentor, and develop management staff while fostering accountability and professional growth.
  • Manage multiple priorities independently in a fast-paced, high-acuity environment.
