About The Position

A 383-bed Level II Trauma acute care hospital in Bradenton, Florida is seeking a Director of Case Management to lead hospital-wide case management and social work operations. This leadership role reports to the Division Director of Case Management and oversees 10+ FTEs. This facility treats over 76,000 patients annually and requires a strong RN leader with demonstrated expertise in length of stay (LOS) management, discharge planning optimization, interdisciplinary coordination, and regulatory compliance. The ideal candidate will serve as the operational “quarterback” for case management initiatives and patient throughput strategy.

Requirements

  • Bachelor’s Degree
  • Active Registered Nurse (RN) license (RN strongly preferred)
  • 3+ years of acute care hospital case management experience
  • 2+ years of case management leadership experience
  • Demonstrated success managing LOS processes and throughput initiatives
  • Recent acute care experience

Nice To Haves

  • Master’s Degree in Nursing, Health Administration, or Business Administration
  • Experience in mid-to-large acute care facilities (300+ beds preferred)
  • Strong executive presence and dynamic leadership style

Responsibilities

  • Direct and evaluate all department operations, including staffing models, onboarding, competency validation, and technology utilization.
  • Oversee all core case management functions including utilization review, discharge planning, care coordination, and documentation integrity.
  • Lead LOS reduction initiatives and drive throughput performance improvement strategies.
  • Allocate resources to meet productivity and quality benchmarks.
  • Partner with the Facility CFO and Division CM Leader to achieve operational and financial targets.
  • Monitor performance metrics, analyze data, and implement sustainable performance improvement initiatives.
  • Ensure compliance with hospital policies, regulatory standards, and accreditation requirements.
  • Collaborate with CMO, CNO, ancillary services, Ethics & Compliance, Legal, and community partners.
  • Supervise Case Management and Social Work teams.
