About The Position

The Director of Case Management is responsible for the overall leadership, management, and coordination of hospital case management operations. This role ensures compliance with organizational policies and regulatory requirements while integrating patient care coordination and discharge planning processes across hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization. The Director provides executive-level leadership, oversees daily case management operations, and is accountable for achieving performance and quality outcomes through interdisciplinary collaboration. This role requires strong leadership presence, strategic thinking, and expertise in acute care case management.

Requirements

  • 3+ years of recent (within the last year) acute hospital case management experience
  • 2+ years of leadership experience
  • Bachelor’s degree (required)
  • Active licensure as one of the following: Registered Nurse (RN) Licensed Clinical Social Worker (LCSW) Licensed Master Social Worker (LMSW)

Nice To Haves

  • Master’s degree in Nursing, Health Administration, or Business Administration (preferred)

Responsibilities

  • Direct and evaluate departmental operations, including case management models, staffing plans, onboarding, competencies, and use of information systems.
  • Allocate resources effectively to meet productivity, quality, and performance goals.
  • Provide oversight of core case management functions and professional practice standards.
  • Ensure compliance with care coordination and discharge planning processes, including timeliness, quality, and documentation accuracy.
  • Partner with finance and divisional leadership to achieve operational and financial goals.
  • Assess department performance by analyzing data, identifying gaps, and implementing sustainable improvement initiatives.
  • Ensure compliance with organizational policies, procedures, and regulatory standards.
  • Build strong working relationships with key stakeholders including executive leadership, nursing, physicians, ancillary services, ethics, compliance, and legal teams.
  • Lead change management initiatives and foster a collaborative, high-performing team culture.
  • Perform additional duties as assigned while adhering to organizational mission, values, and code of conduct.
