The Assistant Director of Case Management is responsible for representing North Spring as the provider for resident services that promote coordinated, effective treatment by facilitating the communication exchange between referral sources, physician/treatment team, and payers. Additionally, this role manages timely transitions through the phases of residential care. Location: Beautiful Leesburg, Virginia, 40 miles Northwest of Washington DC. Requirements

  • Bachelor's degree in human services.
  • Three to five years of relevant experience.
  • Current driver's license with a good driving record.
  • CPR and First Aid/AED certification.

Responsibilities

  • Facilitating communication regarding resident treatment process and the resident’s needs with referral sources and payer.
  • Scheduling and facilitating all treatment team reviews and ensure documentation meets Virginia Medicaid, Tricare and Joint Commission standards. This position is tasked with ongoing compliance of treatment plans.
  • Assures and tracks the timely completion of Certificates of Need for each patient.
  • Coordinates all parties involved in the resident’s care on admission, continued stay and discharge plans.
  • First line of contact and ensures a positive relationship with external and internal customers.
  • Travels to resident’s community meetings representing the facility at court hearings, Family Partnership Meetings, and FAPT/CAT/MDT meetings as needed.
  • Ensures continued availability/recertification of financial resources throughout the residents stay.
  • Providing a resource for other clinical case manager and offering support and guidance in case review.
  • Updating and maintaining Pre-Certification, Continued Stay and Discharge authorization through the Resident Authorization Tracking log and Midas.
  • Facilitating weekly Discharge Planning Meeting.
  • Maintain tracking of CANS to ensure certifications are up to date.
  • Performs other duties as assigned/required by this position.
