About The Position

A large acute care hospital is seeking an experienced Director of Case Management to provide executive-level leadership and operational oversight for all hospital case management functions within a high-volume inpatient setting. This role is responsible for advancing care coordination, optimizing discharge planning, ensuring regulatory compliance, and driving effective clinical resource utilization. The Director will lead a team of approximately 31 FTEs and work collaboratively with interdisciplinary leadership to support quality outcomes, patient throughput, and financial performance. Recent acute care hospital case management experience and prior leadership experience within a comparable-sized facility are required.

Requirements

  • 3+ years of acute care hospital case management experience (Required)
  • 2+ years of case management leadership experience (Required)
  • Recent acute care experience within the past year (Required)
  • Prior leadership experience within a comparable-sized acute care facility
  • Bachelor’s Degree (Required)
  • Registered Nurse (RN)
  • Licensed Clinical Social Worker (LCSW)
  • Licensed Master Social Worker (LMSW)
  • Proven ability to deliver measurable operational and quality outcomes
  • Strong operational, financial, and analytical skills
  • Executive presence with the ability to influence senior stakeholders

  • Master’s Degree in Nursing, Health Administration, or Business Administration (Preferred)

Responsibilities

  • Department Leadership & Operations Direct and evaluate all case management operations, including care coordination and discharge planning
  • Optimize staffing models, including skill mix and FTE allocation
  • Oversee onboarding, competency validation, and staff development
  • Ensure effective use of information systems and data analytics
  • Clinical Oversight & Compliance Ensure adherence to discharge planning and care coordination standards
  • Maintain compliance with hospital policies, SOPs, and regulatory requirements
  • Oversee documentation quality, accuracy, and timeliness
  • Serve as the subject matter expert for hospital case management practices
  • Performance & Financial Accountability Monitor operational and quality metrics to drive continuous improvement
  • Partner with finance and divisional leadership to meet performance and budget goals
  • Implement sustainable performance improvement initiatives
  • Interdisciplinary Collaboration Collaborate with medical staff leadership, nursing leadership, and ancillary departments
  • Lead change management initiatives and foster team engagement
  • Coordinate with internal and external stakeholders to ensure continuity of care
