Requirements

Current and valid license: Registered Nurse in the state of Texas (or compact state) or Masters Social Worker (LMSW) in Texas.
Certification: Certified Case Manager (CCM), Accredited Case Manager (ACM), or Fellowship of the American Academy of Case Management (FAACM) required.
Experience: Minimum five (5) years in utilization management, case management, discharge planning, or other cost/quality management programs.
Demonstrated leadership with at least 5 years of supervisory experience.

Nice To Haves

LCSW preferred.
Three (3) years of hospital-based nursing or social work preferred.

Responsibilities

Drive the implementation and execution of the inpatient case management program, overseeing Care Facilitation, Utilization Management, Case Management, and Discharge Planning.
Ensure compliance with industry standards, regulations, and best practices, optimizing patient care quality and cost-effectiveness.
Collaborate closely with interdisciplinary teams to streamline processes, enhance patient outcomes, and facilitate seamless transitions of care.
