About The Position
The Director of Case Management coordinates the hospital-wide case management program and oversees review of patient medical records to ensure appropriate utilization of hospital services. The role supports case managers in timely care coordination and discharge planning, and ensures all admissions are screened to determine the appropriate level of care. The Director supervises case managers and oversees daily department operations to support optimal financial and clinical outcomes. The position also includes fiscal planning, regulatory compliance, analyzing systems and variances to identify improvement opportunities, and promoting quality care in collaboration with the interdisciplinary team.
Requirements
- RN licensure in the state where the hospital resides is required. Social workers and certain other licensed professionals may be accepted in lieu of RN licensure in accordance with state regulations.
- Current BLS certification is required.
- Three to five years of case management experience is required.
Nice To Haves
- BSN or MSN.
- Certification in an approved Case Management Program.
- Prior experience in IRF or LTACH.
- Prior management experience.
