About The Position

As the Director of Care, you will contribute to Extendicare’s purpose of Helping People Live Better by leading a high-performing care team dedicated to ensuring residents receive the care they need. The Director of Care (DOC) is a skilled and compassionate leader responsible for overseeing all aspects of clinical and care operations within a long-term care home. This role ensures the delivery of exceptional resident-centered care in alignment with Extendicare standards, accreditation requirements, and provincial regulations. The DOC inspires and develops an engaged team of care professionals, fostering a culture of excellence, accountability, and continuous improvement. Building trust-based relationships with residents and families is essential to enhancing resident satisfaction and fulfilling our mission of providing people with the care they need, wherever they call home.

Requirements

  • Bachelor’s Degree in Nursing required; Master’s Degree preferred.
  • Current and unrestricted registration as a Registered Nurse (RN) with the applicable provincial regulatory body.
  • Minimum of 5 years of nursing experience, including at least 2 years in a leadership role within a long-term care home or healthcare setting.
  • Strong knowledge of long-term care regulations, standards, and best practices.
  • Demonstrated leadership, communication, and interpersonal skills.
  • Proven ability to collaborate effectively with interdisciplinary teams and build strong stakeholder relationships.
  • Experience leading quality improvement initiatives and achieving positive resident outcomes.
  • Proficiency with electronic health records (EHR) systems and related healthcare technologies.

Responsibilities

  • Champion resident safety and continuous quality improvement initiatives.
  • Achieve high levels of resident and family satisfaction as measured through experience survey results.
  • Meet or exceed established quality and clinical performance indicators through effective execution of clinical programs.

  • Ensure compliance with all regulatory, organizational, and clinical care requirements, policies, and procedures.
  • Lead and support the resident care team through effective coaching, mentorship, and day-to-day guidance.

  • Ensure care staff complete required education and competency training.

  • Support budget development and monitor departmental financial performance through variance analysis.
  • Monitor and ensure compliance with healthcare regulations, licensing requirements, accreditation standards, labour laws, and organizational policies.
  • Conduct timely investigations and implement corrective action plans as required.
  • Identify and address resident care concerns and clinical risks.
  • Ensure immediate response to critical incidents and compliance with all reporting requirements.
  • Demonstrate a resident and family-centered approach by responding to feedback, concerns, complaints, and recognition with empathy and professionalism.
  • Promote a culture of service excellence among all team members.
  • Oversee resident assessment, care planning, and implementation processes.
  • Ensure individualized care plans reflect resident preferences, needs, and goals.
  • Partner with interdisciplinary teams, residents, families, and leadership colleagues to optimize resident outcomes and care delivery.
  • Lead and implement quality improvement initiatives that enhance resident safety, wellness, and satisfaction.
  • Monitor outcomes and leverage data to drive continuous improvement.
  • Develop and execute operational plans related to clinical services and staffing that support the home's strategic priorities.
  • Build and maintain effective relationships with vendors and community health partners.
  • Collaborate on quality initiatives, training opportunities, and programs that benefit resident care.
