About The Position

The Director of Care Transition develops, coordinates, directs, and administers facility's marketing and public relations programs and services so as to ensure full occupancy. This role is responsible for promoting the facility and its services to outside sources such as physicians, discharge planners, business members of the community, churches and schools providing adult education. The position requires maintaining a thorough knowledge of the facility's admission requirements, services, programs, and policies and procedures. Additionally, the Director will conduct tours for community members and families, perform pre-admission interviews, and explain all services of the nursing facility. This role also involves ongoing communication with the media and developing and updating media contacts.

Requirements

  • 2-4 years' experience in marketing/public relations in a health care setting required.
  • Must possess a thorough knowledge of principles of effective communication, mass media, advertising, community relations, etc., as they relate to nursing facility operations.
  • Ability to make direct sales calls.
  • Ability to organize and deliver public presentations.
  • Must possess ability to establish, implement and maintain effective marketing and public relations program.

Nice To Haves

  • Previous supervisory experience preferred.
  • Bachelor's degree from accredited college with specialization in marketing, communications or equivalent preferred.

Responsibilities

  • Develops, coordinates, directs, and administers facility's marketing and public relations programs and services to ensure full occupancy.
  • Plans, develops, organizes, implements, evaluates, and directs the facility's marketing and public relations programs and activities to maintain and increase census.
  • Promotes the facility and its services to outside sources such as physicians, discharge planners, business members of the community, churches, and schools.
  • Maintains a thorough knowledge of facility's admission requirements, services, programs, and admissions policies and procedures.
  • Conducts tours for members of the community and families.
  • Conducts pre-admission interviews with prospective residents/guarantors and explains all services of the nursing facility.
  • Maintains ongoing communication with the media and contacts them when new programs or promotions are implemented.
  • Develops and regularly updates the current file of media contacts.
  • Keeps the Administrator informed of media accounts that may impact the facility or its regulations.
