The Director of Care Management has a primary focus of leading, directing, mentoring, and supporting the Care Management caregivers and associated processes while collaborating with interdisciplinary teams throughout the health system and managing programs that facilitate the coordination of patient care throughout the continuum. The leader implements/maintains best practice workflows, policies, procedures, caregiver orientations and mentorship, staffing, and overall utilization management structure to ensure caregivers are engaged, represented in department decision making, and are subject matter experts; the patient is receiving the right care, at the right time, in the right setting, and in a cost effective way; and operational outcomes are measured, reported, and optimized. The Director of Care Management collaborates directly with Physician Advisors, Insurance companies, Post-acute care facilities and providers, Finance, Revenue Cycle, Nursing, Registration, Providers and other key team members.
