About The Position

The Director, Integrated Housing and Health Services provides strategic and operational leadership across a portfolio of housing and community-based programs. Grounded in Housing First principles, this role advances an integrated model of housing, health, and psychosocial support for clients experiencing increasing complexity. The Director works across programs, disciplines, and systems to strengthen care coordination, improve outcomes, support housing stability, and ensure high-quality, person-centred services for high-acuity populations. This role reflects the continued evolution of housing leadership toward a more integrated, complex-care model, while maintaining a strong commitment to Housing First, harm reduction, trauma-informed care, and culturally safe practice.

Requirements

  • 7–10+ years of progressive leadership experience in housing, health, social services, or a related sector.
  • Experience leading multidisciplinary teams.
  • Experience managing complex community-based or supportive housing programs.
  • Proven ability to plan and manage a $10M Budget.
  • Operational oversight experience.
  • Experience leading change in high-acuity environments.
  • Knowledge of Housing First, harm reduction, trauma-informed care, and culturally safe practice.
  • Strong systems thinking.
  • Strong communication skills and ability to build and maintain effective internal and external relationships.
  • High level of integrity and dependability, with a results-oriented approach.
  • Knowledge of EMR systems, structure, and contents, along with the ability to analyze and critically evaluate the data.
  • A satisfactory vulnerable sector criminal record clearance and child welfare intervention check are required.
  • Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action.

Nice To Haves

  • A graduate degree in Social Work, Nursing, Health Administration, Public Health, Human Services, or a related field is preferred. An equivalent combination of education and senior leadership experience may be considered.
  • Experience in primary care is considered an asset.

Responsibilities

  • Lead the evolution of housing programs toward an integrated housing and healthcare model.
  • Align services with organizational priorities in trauma-informed, culturally safe, and team-based care.
  • Identify and advance opportunities for integration with primary care, mental health and addictions services, and acute care and hospital systems.
  • Contribute to strategic planning and system-level partnerships.
  • Provide operational and program leadership across a diverse portfolio, including housing programs and teams.
  • Apply strong knowledge of Intensive Case Management models, supportive housing operations, and the integration of housing and healthcare to improve client outcomes.
  • Ensure housing stability and program integrity.
  • Oversee service delivery models, staffing structures, and workflows.
  • Monitor program performance, quality, risk, and outcomes.
  • Manage budgets, contracts, and resource allocation.
  • Lead program design, implementation planning, and continuous improvement.
  • Advance integration of health and psychosocial supports within housing programs.
  • Support effective collaboration between housing teams, RNs, primary care, mental health clinicians, outreach workers, and other clinical partners.
  • Ensure teams have clear pathways for responding to medical complexity, mental health concerns, substance use, overdose risk, and escalating client needs.
  • Promote interdisciplinary care planning, role clarity, documentation standards, and appropriate escalation processes.
  • Partner with the Director of Primary Care, medical leadership, and other clinical leaders to support safe, coordinated, evidence-informed care.
  • Lead and support program managers and team leads.
  • Foster a culture of accountability, collaboration, and psychological safety.
  • Support teams in managing complex client situations.
  • Develop leadership capacity across programs.
  • Oversee client safety and critical incident management.
  • Strengthen systems for client safety, staff safety, critical incident review, risk mitigation, and learning from adverse events.
  • Monitor key indicators related to housing stability, client outcomes, service utilization, quality, and access.
  • Use data, client experience, staff feedback, and incident trends to inform planning and improvement.
  • Lead continuous improvement initiatives that strengthen consistency, integration, and service quality across programs.
  • Build and maintain relationships with health system partners, funders, and community agencies.
  • Represent the organization in cross-sector initiatives.
  • Advocate for integrated approaches to housing and health.
  • Perform additional duties as assigned.
