About The Position

Emplify Health is seeking a dynamic and experienced Director of Community Health & Wellbeing to join our enterprise management team. Reporting to the Vice President, Community Health & Wellbeing, this leadership role is responsible for directing community health improvement and benefit activities, employee and community wellness programming, population health initiatives including community paramedicine, and ensuring alignment with our strategic plan to foster a thriving community. The Director will build infrastructure to enhance program effectiveness, financial outcomes, customer satisfaction, and market growth while attracting and retaining top talent to fulfill our mission.

Requirements

  • Bachelor’s degree in public health, healthcare administration, community health education, business administration, or related field.
  • 5-7 years in a management position within healthcare or business administration.

  • Master’s degree in public health, healthcare administration, community health education, business administration, or related field.

Responsibilities

  • Lead development and execution of data-driven initiatives to improve health outcomes for targeted populations based on the Strategic Plan, Community Health Needs Assessment (CHNA), and other data sources.
  • Oversee the Community Health Implementation Plan.
  • Ensure compliance with Federal, State, and local laws related to community benefit regulations, employee wellness programming, and community paramedicine.
  • Collaborate with Finance on community benefit reporting and regulatory compliance.
  • Monitor healthcare environment trends to identify opportunities and lead implementation of population health advancements.
  • Develop and maintain programs addressing population health priorities for patients, community members, and employees.
  • Provide leadership on initiatives addressing social determinants of health.
  • Advocate for health improvement through organizational and public policy influence at local, state, and federal levels.
  • Integrate population health priorities into organizational strategic planning.
  • Build and sustain external stakeholder relationships through partnerships, committee memberships, and community engagement.
