The population health team within Mount Sinai encompasses payer contracting, population health analytics and operations, care management, wellness services, provider communications and engagement, clinical integration, and population-based quality and efficiency management and improvement. The team includes 400+ employees with clinical, contracting, finance, IT, analytics, operations, and product development expertise. MSHP is a fast growing business unit within Mount Sinai and is looking for team members who: Are comfortable “playing up” and “playing down” as needed to accomplish business objectives Work productively amidst ambiguity Thrive in fast-paced work environments Seek to improve the status quo Within MSHP Clinical Operations, the Clinical Integration Team drive value-based contract performance by decreasing unnecessary variance on leading clinical practices and decision making (i.e. processes), establishing and disseminating measurement and transparency of the achievement of leading clinical practices and adherence to key care milestones, assure provider engagement and alignment to support clinician workflow and assure linkages among diverse programs and resources.
