About The Position

Heartline Staffing, strategic talent partners with Purple Cow Recruiting, is seeking an experienced Director of Case Management to lead hospital-wide case management and discharge planning operations within an acute care setting. The Director of Case Management is responsible for managing and coordinating all aspects of the hospital’s case management department. This leader ensures compliance with organizational policies and regulatory requirements while integrating patient care and discharge planning processes across hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization. This role provides executive oversight of daily case management operations and is accountable for achieving established quality, throughput, and utilization outcomes. The Director actively engages interdisciplinary teams and external stakeholders while maintaining a patient-centered approach to care coordination and discharge planning. The ideal candidate will bring strong executive presence, excellent communication and presentation skills, and a proven record of motivating teams and delivering measurable operational results in an acute care environment.

Requirements

Bachelor’s degree required
Master’s degree in Nursing, Health Administration, or Business Administration preferred
Active Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) license required
Minimum of 3 years of acute care hospital case management experience
Minimum of 2 years of leadership experience in hospital case management
Hospital discharge planning experience required
Recent acute care experience required
Executive presence with strong communication and presentation skills
Demonstrated success leading interdisciplinary teams
Proven ability to deliver operational, quality, and throughput results
